Your household income is at or below 135% of the Federal Poverty Guidelines. Documentation such as tax returns or paycheck stubs may be required.
You participate in certain government assistance programs such as Medicaid, SNAP (Food Stamps), SSI, Federal Public Housing Assistance, or Veterans Pension.
Only one Lifeline service is available per household. Applicants must be at least 18 years old or an emancipated minor.
You'll need to provide proof of identity, address, and program participation or income eligibility when applying for the service.
